Overview: Readmissions are a topic of much discussion right now, and there are a number of high profile efforts to reduce them. The impact of Medicare readmission penalties has hospitals across the country focusing on strategies to reduce readmissions.

Learning Objectives: After completing this course you will be able to:

  • The financial implications of reducing readmissions including utilizing resources already in place and reallocating other resources
  • How to use data from the QualityNet Hospital-Specific report to identify and understand current readmission patterns of the 5 core measures used to calculate the HRRP penalty
  • Operational initiatives including the roles of the patient, physician and care team in mitigating readmissions that may not be medically necessary
  • How to develop a comprehensive readmissions tool kit to address medication reconciliation, patient/family education, provider communication and risk stratification

Prerequisites: Attendees should have a basic knowledge of hospital readmissions penalties and hospital operations
